Blytheco Employee Benefits Summary

 

Health Benefits
Benefits provided by BlueCross.  Benefits begin on the first of the month following 30 days of employment. Blytheco offers Health, Dental, Vision, LTD and Life Insurance benefits for all eligible employees. 

Medical coverage is available in both HMO (California only) and PPO options.  Blytheco provides a defined contribution for each eligible employee toward the cost of Health, Dental and Life insurance benefits.

 

401(k) Savings and Retirement Plan
Employees are eligible to contribute to the 401(k) plan following their eligibility period (first of the month following 30 days of employment). Open enrollment is January 1st, April 1st, July 1st and October 1st of each calendar year. Employees may contribute to the 401(k) plan by deferring from 1% to 100% of compensation on a pre-tax basis up to an IRS maximum.

 

Personal Time Off (PTO)
All full-time employees are eligible to receive PTO, which may be used for vacations, sickness, religious holidays and observances, and personal emergencies. Personal hours/days begin accrueing immediately upon your first day of employment.

Medical: BlueCross (HMO (California only) and PPO plans available).
Chiropractic coverage is included in all plans.
Employer pays 80% of employee premium.
Flu Shots (annual and on-site) Employer Paid
Dental: Guardian (DHMO Plan 75 (California only), PPO Plan 1500 and PPO Plan 2000) Employer pays 50% of employee DHMO premium.
Vision: BlueCross (BlueView) Voluntary plan
Ft. Dearborn Life Insurance $25,000 benefit per employee
Ft. Dearborn Long Term Disability (LTD) Insurance Employer paid plan
Short Term Disability Insurance (SDI) Yes
401(k) Plan (Quarterly Open Enrollment) Yes

Paid Time Off (PTO) Dependent upon years of service
(additional seniority accrual level of 5 additional days at year 5 and year 10)
